If I am not in-network with your insurance plan, I can provide a superbill for you to submit directly to your insurance company for potential out-of-network reimbursement.

Please note that reimbursement is not guaranteed, and I encourage you to verify your insurance benefits before we begin our work together.

1. Do I have coverage for outpatient nutrition counseling for the following codes?

• Procedure codes: 97802 (initial nutrition assessment) and 97803 (nutrition counseling follow-up visit)

• Diagnostic code: Z71.3

• If applicable: Do I have different coverage with an eating disorder or diabetes diagnosis (same procedure codes)

2. How many visits are covered per year?

3. Do I have a copay, and if so, how much?

4. Do I need to meet a deductible before I have coverage?

5. Is Eating With Ease Nutrition Counseling in-network with my plan?

Out of Pocket Rates

• Initial Intake Session -75 minutes: $180

• Follow-Up Session 50–55 minutes: $140

• Short Check-In Session 30 minutes: $85

Payment is due at the time of service and the credit card on file will be charged.

Sliding Scale

I offer a limited number of sliding scale spots to increase accessibility of care. Please reach out if cost is a barrier, I’d be happy to discuss current availability.

Cancellations & No-Shows

Appointments canceled with less than 24 hours’ notice, or missed without notice, will be charged the full session fee.
